ICD-9轉換為ICD-10對疾病分類人員造成的工作壓力之探討――以北部某區域教學醫院為例

Explore the ICD-9 to ICD-10 conversion classification of disease caused by work stress personnel - To the north of a Regional Teaching Hospital

目的：1995年我國健保實施後，各層級醫療院所健保病人的疾病與處置分類以ICD-9-CM編碼。1993年WHO公布ICD-10，美國已公告將於2015年10月1日起啟用ICD-10-CM/PCS，我國健保署則規劃於2016年正式實施，ICD-10-CM/PCS的實施對醫界的衝擊將非常大。本文乃探討對於我國健保署實施ICD-10之政策，對疾病分類人員造成之工作壓力。方法：以北區某區域教學醫院擔任疾病分類業務經歷超過4年以上之疾病分類人員為研究對象，以半結構訪談方式進行。資料分析嚴謹度是採四項原則：真實性 (credibility)、可轉換性 (transferability)、可靠性 (dependability) 與可確認性 (conformability)。結果：將疾病分類人員面對壓力考驗的歷程分為三階段，即：摸索新角色、學習扮演新角色以及接受新角色，將訪談錄音檔案轉換成逐字稿之文本，擷取與疾病分類人員對於工作壓力相關描述。在摸索新角色階段總結有以下要點：（一）所學不足以應付現實、（二）無法勝任角色的焦慮、（三）角色轉換的困難、（四）適應新工作的態度與方法；在學習扮演新角色階段：（一）適應作業變更、（二）需要同事與家人的支持；在接受新角色階段：（一）重拾工作熱情、（二）開始思考工作以外事物。結論：忙碌的醫療行政工作環境中，疾病分類人員面對種種考驗由生手轉變為具能力的疾病分類人員過程就已經相當複雜，現今又必需應付ICD-9轉換成ICD-10所會遇到的種種挑戰與困境，且從事疾病分類工作之人員又以女性佔多數，更需面對如何能夠兼顧家庭的照顧的重要課題，本研究忠實呈現疾病分類人員角色轉換過程焦慮與無助的心情。

Purposes: To discuss the implementation of the ICD-10-CM/PCS, what's the pressure for a coder who he/she will be facing? Methods: Coder who he/she is working in the regional hospital at northern Taiwan and he/ she also have 4 years experiences will be selected. The semi-open questionnaire will have four dimensions: credibility, transferability, dependability and conformability. Results: Coder's pressure are divided into three stages: to explore new role, New role in the exploration stage are summarized into the following viewpoints: (a) is insufficient to cope with the reality of what they have learned, (b) easy facing into anxiety, (c) difficult to transection of the role, (d) created an adaptor into the new attitudes. Conclusion: Mostly, the coder are females and they're facing huge pressure when change the coding system from ICD-9-CM to ICD-10-CM/PCS, just like the candle, it's burning at both ends
