降低抗藥性鮑氏不動桿菌交互感染之專案

Improvement of Cross Infection by Drug Resistant Acinetobacter Baumannii

本單位於2010年7月及9月共發生二次病人抗藥性鮑氏不動桿菌交互感染，故成立專案小組。經現況分析確立問題為：一、醫療相關人員對抗藥性菌種照護認知不足。二、感染管制防護措施執行不正確。三、環境清潔不確實。擬定改善方案：一、舉辦在職教育。二、重新規劃病室物品擺放。三、修訂隔離提醒標示及製作衛教看板。四、制訂環境清潔查檢表。五、制訂接觸隔離衛教單。六、增加隔離衣更換次數與圍簾換洗頻率。七、爭取固定的清潔人員等七項，經介入措施後，醫療相關人員抗藥性菌種照護認知正確率由58.5％提升至95.6％、感染管制防護執行正確率由81.7％提升至97.8％，專案期間為2010年10月～2011年12月，且由2012年1月追蹤至2013年10月未再發生交互感染個案。

The unit in July 2010 to September occur the secondary resistance Acinetobacter baumannuu interactive infection, so we set up a group to improve this problem. We establish the problem during analysis were: (1) drug-resistant care of health care personnel cognitive inadequate (2) infection control protocol implement incorrectly. (3) the environment clean is not indeed. To develop improve program as: (1) conduct education. (2) re-setting appliances placed in the ward. (3) revise isolation remind marks and design health education kanban. (4) daw up environmental cleaning checklist. (5) draw up contact isolation education sheet. (6) to increase the replacement gown and confining curtain wash times. (7) try for fixed cleaning staff in unit etc., after the intervention, the correct rate of drug-resistant strains of health care personnel cognitive improved from 58.5% to 95.6%, the correct rate of the infection control protocol implement increased from 81.7% to 97.8%, January 2012 to October 2013, not new case of cross-contamination occur.
