This study is focused on providing an integrated care to patients who have completed treatment for acute illness and examined the overall effectiveness and related factors in patients before and after the intermediate care. 207 patients, from January 2011 to December 2012 with an average age of 79.9±10.1, were selected from the ”intermediate care case data management system” by secondary data analysis. The results show the overall functions of the patients were improved in every aspect after the intermediate care. The quality of life and health condition were also significantly improved. It is observed that the patients' gender affects the quality of life, marital status influences the instrumental activities of daily living and the mental status is affected by their educational status. The intermediate care is a significantly effective service model. It is suggested that this model should, in the future, utilize the existing health care system and long-term care resources and the teamwork of professionals and perform completed systematic assessments to improve the quality of care and to provide post-acute care. The intermediate care should effectively utilize the resources to provide a better and completed health care. |
