應用芳香療法在一位接受血液透析病人之照護經驗

Nursing Experience with Aromatherapy for Patient Undergoing Hemodialysis

本篇是描述一位羅患痛風導致末期腎衰竭需長期血液透析之病人，在於初次接受血液透析治療期間因有體液容積過多，對血液透析、末期腎衰竭疾病過程不瞭解及害怕廔管阻塞，進而使病人對此產生焦慮感。筆者藉由觀察、會談、身體評估等技巧，運用Gordon十一項功能性健康評估該病人有：體液容積過量、焦慮、知識缺失等健康問題，在護理過程中給予指導及反覆示教，並提供疾病相關問題之資訊，讓病人對疾病有正確認知，並鼓勵說出內心想法，協助病人適應疾病治療過程，增加自我照顧能力。另外藉由應用輔助療法中的芳香精油療法，透過按摩、泡澡、吸聞等方式進入體內，以達身、心、靈皆能維持平衡健康及和諧，讓病人學習自我釋放壓力，且適應因疾病所造成的生活改變，以正向的態度面對疾病的衝擊。

This article describes a patient in the final stage of chronic renal insufficiency caused by gout and requires long-term hemodialysis. During the initial treatment hemodialysis treatment, hypervolemia, ignorance about the hemodialysis process, the symptoms of final stage of chronic renal insufficiency and the failure of AV fistula led to patient anxiety. An evaluation was conducted using the methods of observation, communication and physical examination as well as Gordon's Health Pattern Typology. The patient was determined to have problems such as hypervolemia, anxiety and insufficient knowledge. Guidance and repeated instruction were therefore provided during hemodilaysis to teach the patient about the disease and enhance the patient's self-care ability. The patient was also encouraged to express her feelings and thoughts to help her come to terms with the treatment process. During complementary therapies the essential oils in aromatherapy were absorbed through massages, baths and inhalation treatments to keep the patient physically and mentally fit. The patient was shown how to cope with stress, adapt to the lifestyle changes caused by disease and face the impact of illness with a positive attitude.
