協助一位尿毒症病患及家屬接受腹膜透析之護理經驗

Helping a Uremic Patient and His Family Accept Peritoneal Dialysis

本個案報告主要探討一位41歲男性，因患尿毒症而接受腹膜透析治療，個案及案妻其適應過程之護理經驗。筆者於91年4月12日至7月5日期間提供照護，其未住院期間也以電話追蹤訪談提供照護，以Gordon十一項健康功能型態為護理評估工具進行資料收集，經由觀察、會談及護理評估確認個案之護理問題，其護理問題有知識缺失、疼痛及性生活型態改變。在個案住院期間經筆者有計劃的護理措施，以耐心、關懷照護、支持與鼓勵，協助個案及案妻解決問題、走過調適過程，接受與腹膜透析共存之事實。期望藉由此篇個案報告將其照護經驗與大家分享，以提供護理人員日後護理此類病患之參考。

This case report describes the nursing care experience of a 41-year-old male in peritoneal dialysis. The author offered medical nursing service from April 12 to July 5, and kept in touch with the patient by phone when he wasn't hospitalized. The Gordon 11-item Functional Health Pattern served as an assessment tool to evaluate the patient's health problems and collect relevant information through observation, interview, and the nursing process. We found that the patient suffered from knowledge deficit, pain, and altered sexual patterns. During hospitalization, the author adopted every possible nursing method to help the patient undergo the adjustment process. This experience hopefully can become an example for future cases.
