This report describes a nursing experience for a patient with functional impairment secondary to cerebrovascular accident. The nursing period was from January 8 to 25, 2010. The author applied Gordon's 11 functionality healthy state appraisal as the tool for data collection and analysis. The four identified major nursing problems were (1) impaired functional activities related to muscle weakness; (2) anxiety associated with the change in heath status; (3) incapability of self-care due to loss of body function; and (4) altered bowel movement associated with insufficient activity. The author, with full empathy, attentively listened to the patient's needs and provided healthcare information for patient education. Through a good therapeutic relationship with the patient and caregivers, nurses were able to assist the patient in coping with the disease and receiving rehabilitative process. The author shares this nursing experience and hopes it may be used as a reference for the nurse taking care of the patient with stroke in the future. |
