照顧一位塵肺症再度入院個案之護理經驗

A Nursing Experience of a Readmitted Patient with Pneumoconisis

本文旨在照護一位罹患塵肺症患者之護理經驗，個案於2000年診斷為塵肺症，2007年2月因肺炎住院，同年6月再度入院，護理期間為2007年7月5日至2007年7月17日，運用Orem理論，以觀察、筆談、溝通及身體評估等方式收集資料，歸納出護理問題有：一、『呼吸道清除功能失效∕因呼吸道分泌物增加所致』二、『活動無耐力∕因活動呼吸喘所致』；三『營養少於身體需要∕因飲食不均衡所致』四、『無望感∕因疾病照護所致』。照護期間以增加自我照顧為基礎，給予適當且個別性的護理措施，協助個案增加自我管理疾病的信心，及協同家屬共岡參與照護計劃，減少再度入院與提升其生活品質。

This article shared a nursing experience of a patient with pneumoconiosis. The patient was admitted to the hospital for the same health problem in February, 2007, and was readmitted in June, 2007. The nursing care period lasted from July 5 to July 17, 2007. With Orem's theory, the authors collected data through observation, conversation via writing, and physical assessment. Identified problems included: 1. dysfunction of clearance for airway due to increased secretion; 2. poor activity tolerance secondary to respiratory exacerbation; 3. malnutrition associated with diet imbalance; and 4. disease related hopelessness. With the focus of self-care, the authors developed a proper individualized nursing care plan. A boost of the patient's self-confidence in disease management and the patient family’s involvement in the care plan reduced the patient's readmission and improved the patient's quality of life.
