本文是描述於復健病房中照顧一位初次罹患腦中風個案及家屬如何面對疾病恢復期的身心調適之照護經驗,藉由觀察、身體評估及會談等方式收集資料,並運用Gordon十一項健康功能模式評估個案及家屬有身體活動功能障礙、身體心像紊亂、照顧者角色緊張等護理問題。於護理過程中運用關懷與支持,瞭解個案初次罹患腦中風後,對個人及家屬的衝擊,協助個案調適因疾病造成的肢體功能喪失,重建正向的自我概念,同時與家屬共同擬定復健運動計劃,提供疾病相關知識、照護技巧及社會資源的申請,以緩解家屬在照護上的無助。期望透過此個案護理經驗的分享,作為日後照護此類個案之參考,使護理人員獨特角色功能得以發揮。This article reports the nursing experience in assisting a stroke patient and his family in the physical and psychological adaption process during the disease recovery stage. Nursing assessment data were collected based on the Gordon's Eleven Functional Health Patterns. Three health problems were identified, including impaired physical mobility, body image disturbance, and caregiver role strain. Through providing nursing care and support, the patient and his family were able to adjust with the loss of limb function, improve functioning, and regain positive self-concept. A rehabilitation plan was developed in cooperation with the patient and his family. Disease-related knowledge, nursing skills, as well as information regarding community resources were provided to the family to ease their sense of helplessness. It is hoped that this case report can serve as a reference for nurses who take care of similar cases in the future. |
