一位意外燙傷之肝醣儲積症患童及其母親的照護經驗

Nursing Experience of a Child Who Had Glycogen Storage Disease with Burn in an Accident and His Mother

本個案報告目的在探討一位意外燙傷之肝醣儲積症患童及其母親的照護經驗。筆者為外科加護病房護理人員，於95年2月1曰至2月14曰期間擔任其主護護士。護理過程以羅氏適應模式為評估工具，並運用觀察、會談、身體評估等技巧收集資料，發現患童有急性疼痛、皮膚完整性受損、營養不均衡：少於身體需要等健康問題，母親則有照顧者角色緊張的問題。護理過程中，筆者運用角色扮演、遊戲及兒童溝通技巧，鼓勵患童說出內心感受，與患童建立信任感及護病關係；另針對母親的感受及需求，給予支持與協助，以增強其照顧者的角色。盼此護理經驗能提供類似個案之臨床照護參考。The purpose of this case study was to explore a nursing experience of a child who had Glycogen storage disease and burnt in an accident; and the caregiving experiences of his mother. The author was his primary nurse in S.I.C.U. from February 1st to 14th, 2006. The nursing process was based on the framework of Roy Adaptation Model. Data was collected by site observations, interviews and physical assessments. Four health problems were concluded including acute pains, skin integrity impaired, and malnutrition. In addition, the caregiver had serious strains when she took care of him. The author applied role plays and intensive communication skills to encourage the boy to describe his feeling and perceptions during nursing process. These skills helped the author to build relationships with the patient and his mother. Additionally, we also provide appropriate supports and assistance to help the patient’s mother enhance her role as a caregiver. The experience of nursing care is useful as a reference for taking care of similar cases.
