應用紐曼模式於中風病患及其主要照顧者之護理經驗

The Care Experience of a Stroke Patient and His Caregiver Based on Neuman's Model

本文係利用紐曼模式來評估一位中風個案及其主要照顧者的護理經驗。筆者在護理個案期間，運用觀察、 與個案及案妻會談及身體檢查與評估的方式收集資料，再將資料加以歸納與分析，發現個案主要的健康問題有 （一）氣體交換障礙（二）自我照顧能力缺失：進食、沐浴/衛生、穿著/修飾、如廁（三）身體活動功能障礙；另案妻主要的健康問題為照顧者角色緊張。從整個護理過程中，深感護理人員除了針對個案問題提供護理措施外，尚需擔任支持者、聯絡溝通者、諮詢者和傾聽者的角色。期藉此次文獻探討及護理經驗，能提醒護理 人員適當且具個別性護理措施對中風病患及其主要照顧者的重要性，冀望此經驗能作為曰後照顧類似病患之參考。This case study, based on Neuman’s Model, focused on the nursing care of a stroke patient and his caregiver. During the admission period, the collection of data was accomplished by clinical observation, physical assessment, and interviewing of the patient and his caregiver. Major health problems of the subject were found, such as impaired gas exchange, self-care deficit, and impaired of physical mobility. The major health problem of the caregiver was caregiver role strain. During the nursing process, the nurse not only took care of the physical needs of the patient, but also played the role of supporter, communicator, consultant and listener. To provide suitable and individual intervention is very important for the stroke patient and primary caregiver. The study will be of interest to clinical nurses as a reference in caring for similar patients.
