一位直腸癌末期患者之護理經驗

The Nursing Experience of a Patient with End-Staged Rectal Cancer

本篇個案報告為一位末直腸癌末期合併胸椎轉移病患之護理經驗。筆者於2007年4月10日至5月11日照護期間，透過直接照護、運用身體評估、觀察及會談等技巧收集資料，經由分析歸納出個案的建康問題（1）高危險性感染、（2）疼痛、（3）遺憾牽掛、（4）找不到現存的意義與價值等護理問題。護理過程中筆者運用安寧療護的理念及Watson關懷理論為基礎，提供個案及家屬緩和與支持的醫療照護及協助個案面對內心真實感受等措施，協助個案處理生、心、社會及靈性層面的問題，促使個案在護理過程中能安詳、有尊嚴地走完人生最後旅程，達到善終的境界。This article reported the nursing experience of a patient with terminal phase rectalcancer and thoracic spinal metatasis. I cared for the patient with direct personal nursing care, using physical evaluation, 0bservation and conversation from April 10 to May11,2007. Results showed that the patient had nursing problem including, high risk for infection, pain, spiritual perspectives. In the process of nursing I used palliative care and Watson's theory. I provided her and family with medical information. I took care of her needs in a timely manner. Used physical care to make her fell comfortable, and helped her to manage physiological, psychological social and spirtual problems.I enable the patient had a good death. in the hope that she would experience the end of her life peacefully under our professional nursing care.
