協助一位接受體外震波碎石術患童及其母親之護理經驗

An Nursing Experience of Caring a Patient with Extracorporeal Shock Wave Lithotripsy

本篇個案報告旨在協助一位13歲輸尿管結石之患童及其母親接受體外震波碎石術之護理經驗。護理期間自2008年2月17日至2008年2月19日運用戈登（Gordon）十一項評估，藉由觀察、會談、身體評估等方式收集資料，確立個案主要護理問題為急性疼痛、焦慮、潛在危險性感染及案母出現照顧者角色緊張等護理問題，運用主護護理關懷、接受、支持技巧與個案建立良好的護病關係，提供個案及案母體外震波碎石術護理資訊與指導，協助減輕個案的疼痛和焦慮，病人於術後48小時沒有尿路感染及併發症發生，同時也提升案母對患童結石的了解及相關照護能力。此護理經驗可做為護理人員照護之參考。In this report, we explored the nursing experience of caring for a 13 year-old child who received extracorporeal shock wave lithotripsy(ESWL).The nursing care was provided from Feburary 17, 2008 to Feburary 19, 2008. The patient was assessed with the Gordon's functional health assessment tool. Through conversaton, observation, and physical examination, the problems were identified. They included acute pain, anxiety, risk for infection, and stress the patient's mother undertook as a caregiver. During the period of primary nursing care, the nurse established a good nurse- patient relationship by listening to the patient and family's concerns with acceptance and support. The nurse assisted the patient in reducing acute pain and anxiety. The nurse provided the patient and family with preoperative information and postoperative ESWL care. The family become more confident in handling the patient's care needs. The patient did not have urinary tract infection or other complications within 48 hours after ESWL. We hope that this study would be a valuable clinical learning experience shared with the nurses caring for patients with ESWL.
