提升精神科居家照護之服務量

Enhancing the Capability of Medical Team in Managing Aggressive Events in Acute Psychiatric Wards

本專案旨提昇精神科居家照護服務之服務量。本單位自1996年成立居家照護已超過10年，由於居家照護服務只提供某榮家病人，故服務量停滯在每月約13人次，缺乏醫療服務之廣泛性。小組針對曾有居家業務經驗之醫療人員問卷調查後，分析主要原因為缺乏專職人員、對居家業務認知不足、未依標準作業流程進行、服務範圍限某榮家；經設立專職居家護理師、定期舉辦在職教育、制定標準化表單、修訂標準作業流程、擴大服務範圍至社區等措施後，居家照護服務量由13人次提昇至32人次，專案目標達成。此專案可為其他尚未成立居家照護之醫療院所在規劃時之參考。

This project was aimed to increase the service volume of psychiatric home care. Our department launched a home care service over 10 years ago, since 1996. As our home care service was limited to patients of a veteran home, the total of service recipients was held at around 13 person-times each month over a long period, thus lacks generality. According to the analysis of the questionnaires done by medical staffs experienced in home care, the main causes were the lack of full-time personnel, insufficient knowledge on home care, violation of the standard operation procedure, and the restriction of service in one veteran home. Our approaches included training a full-time nurse specializing in home care, holding regular on- job training, formulating a standardized list, revising the standard operation procedure and expand service areas into the communities. Subsequently, the numbers of home care recipients rose from 13 person-times to 32 person-times per month, thus achieving the goal of this subject.This project may be used as a reference to other hospitals that are in the process of establishing a psychiatric home care project.
