急診室的病患具有科別複雜、疾病嚴重度高、異動頻繁的特性;急診護理人員在急救處置穩定患者病況後,與病房護理人員交班的正確性及完整性,關乎患者是否能被安全送抵病房來接受到完善的後續治療與照護。因不良事件申訴與護理同仁爭執,為增進某急診病患轉送病房護理交班完整性及病房護理人員對單位交班滿意度,故實施此專案。過程中建立了急診病患轉送病房交班標準,來提昇急診護理交班完整性與服務品質。於2005年7月至2006年4月,制訂急診病患轉送病房交班單,執行並定期查核期間急診病患轉送病房交班完整性,結果由59%提昇至91%;病房護理人員對急診護理人員交班滿意度由55%提昇至88%,目前仍持續進行中,更期望能推廣至急診送檢與院外轉送部分,使急診病患安全能更有保障。
英文摘要
Emergency department (ED) patients are likely to be at high acuity level, have complex diseases, and are more frequently transferred within or between institutions. After initial stabilization in the ED, these patients are often transferred to the ward for continued care and treatment. A safe transfer depends on accurate and detailed communication between the ED and ward nursing staff. We launched a program to reinforce the completeness of the transfer process and to fulfill expectations from ward nurses. This program established standard guidelines for transferring patients from the ED to the ward. From July 2005 to April 2006, a transfer checklist was utilized and the completion rate increased from 59% to 91%; meanwhile, the satisfaction level of ward nurses increased from 55% to 88%. This ongoing program is expected to be applied to ED patients who are transferred to other departments for examination, or to other hospitals.
