Emergency department (ED) patients are likely to be at high acuity level, have complex diseases, and are more frequently transferred within or between institutions. After initial stabilization in the ED, these patients are often transferred to the ward for continued care and treatment. A safe transfer depends on accurate and detailed communication between the ED and ward nursing staff. We launched a program to reinforce the completeness of the transfer process and to fulfill expectations from ward nurses. This program established standard guidelines for transferring patients from the ED to the ward. From July 2005 to April 2006, a transfer checklist was utilized and the completion rate increased from 59% to 91%; meanwhile, the satisfaction level of ward nurses increased from 55% to 88%. This ongoing program is expected to be applied to ED patients who are transferred to other departments for examination, or to other hospitals. |
