一位糖尿病足合併左膝下截肢病人呼吸器脫離之護理經驗

The Nursing Experience for a Diabetes Mellitus Foot with Left Below Knee Amputation Patient Weaning from Ventilator

本文敘述一位因糖尿病足部病變而行左膝下截肢術病人進行呼吸器脫離之護理過程。本篇個案因脫離呼吸器多次失敗，加上左膝下截肢而出現漠視身體外觀改變之行為及沮喪無助的負面自我觀點，筆者藉由病人觸碰、正視患肢，鼓勵家屬一起參與治療過程，共同努力改善病人的呼吸型態，進而脫離呼吸器，協助控制本身的疾病，以提昇病人的自我價值感。透過提供病人個別性、連續性之身心照顧，使其在生理、心理、社會與靈性方面得到良好的調適，並促進自我照顧能力及改善生活品質，亦期望藉此經驗，提供護理人員照顧此類病人之參考。

The purpose of this report was to share the nursing experience for a diabetes mellitus foot with left below knee amputation patient weaning from ventilator. The patient was many times experience for weaning failure, post operation of amputation, she had upset and negative ego standpoint by the body image change. We help and teach the patient to touch and suffer the operation limb, in the process, family members also participate together. We improves the patient's breath type and weaning from ventilator successfully, Help the control oneself of disease, promote the patient's ego value feeling, make the patient get the good adjustment in the physiology, mental state, society and innate intelligence,promote ego care ability and improvement quality of life. We also expect to provide the nursing personnel to look after the reference of this kind of patient by this experience.
