急性胰臟炎病患之護理經驗－－The Synergy Model之應用

The Nursing Experience of Caring for An Acute Pancreatitis Patient: Application of the Synergy Model

急性胰臟炎是消化系疾病中，一種常見、會對生命造成潛在威脅的疾病，有20％會出現壞死性或嚴重性胰臟炎，會伴隨高程度的死亡率。本篇個案報告是探討一位急性胰臟炎病患，出現胰臟壞死及併發症而轉入內科加護病房的護理經驗，護理過程自2008年11月6日至12月2日，運用同步模式(the synergy model)中的8項病人特性來評估與判斷個案的需要，利用8項護理能力給予最合適的照護，提供重症病患高複雜性疾病的完整照護，展現護理特質，以符合個案需求，使得個案能藉由此整合模式的護理下，讓重症疾病能趨於穩定，提升個案的預後，讓個案能度過疾病的急性期，轉入呼吸治療中心，進行呼吸器脫離計畫的後續照顧。

Acute pancreatitis is a common and life-threatening disease of digestive systems. About 20% patients will progress into necrotic pancreatitis with a high mortality rate. The paper reports the nursing experience of a patient who had necrotic pancreatitis and complications in a medical intensive care unit from November 6 to December 2, 2008. The synergy model was applied to assess the complex patient situations based on 8 patient characteristics and to provide comprehensive nursing care based on the 8 nurse competences. The synergy model helps nurses to improve the patient outcome. The patient became stable and was transferred to respiratory care center for weaning off of the ventilator.
