照護一位青少年罹患腦中風之護理經驗

An Experience of Nursing Care for an Adolescent Suffered From Stroke

本文描述一位16歲青少年參加學校拔河活動時突發腦中風，於病情穩定後因右側肢體功能缺損及言辭表達障礙，入院接受復健之護理經驗。護理期間自2008年2月11日至3月29日，透過訪談及實地觀察收集資料，運用羅氏適應模式進行評估及分析，結果發現個案有「身體活動功能障礙」、「言辭溝通障礙」、「社交互動障礙」等健康問題，筆者引導個案自我執行全關節運動、擬定復健計畫及說話訓練，透過聯繫院內志工重新建立新的同儕關係，使個案能適應新生活，期望藉此照護經驗提供護理人員對類似個案之照護參考。

This report described an experience of nursing care for a 16 year-old adolescent having a stroke when attending a tug-of-war activity in the school, then receiving rehabilitation therapy in the hospital due to the occurrence of right limb movement and verbal expression dysfunctions. During the period of nursing care, from February 11, 2008 to March 29, 2008, the author collected the information through interview and observation of the case. The Roy’s adaptation theory was used to analyze the information. Finally, we found that the case had problems such as “dysfunction of physical activity”, “disturbance of verbal communication” and “defect of social interaction”. During the period of nursing care, the authors helped the case to do whole-skeletal exercise by himself and drew up the rehabilitation plan and verbal training. The authors also assisted the patient with reconstructing a new peer relationship through connecting with the social workers or volunteers in the hospital and made the case adapt to the new life. It is hoped that this nursing care experience can provide the nursing staff as a reference when taking care of such cases.
