Most of the patients admitted to Cardiology ward are elderly. They are at a high risk of falling down due to advanced age. The study population was recruited from May 1, 2007 to April 20, 2008. The current analysis revealed that patients with falling risk screening score ≥1 were categorized to high risk group. The execution rate of falling prevention measures offered by nursing staff was 60%. Awareness of falling from patient and their major caregivers was 69%. Measures for improvement included: 1.Set up a standard protocol to prevent hospitalized patients from fall; 2.Develop a special sheet for turning over to the next shift about the high risk patients; 3.Design a reminding card for fall prevention; 4.Provide a La-La wristlet; and 5. Arrange education programs. The result of this study showed that the percentage of falling dropped from 0.12% to 0.02%, which has reached the goal of 0.06 set by the program. The execution rate of falling prevention measures offered by nursing staff increased from 60% to 95%. The awareness of falling among patients and there major caregivers increased from 69% to 84%. Although the completion rate of 85% was not achieved, the total elevation was up 15%. The study result showed effectiveness of this project. Besides, the objectives of patients’ safety improvement and nursing quality promotion were achieved. |
