運用羅氏模式於一位運動神經元疾病個案之居家護理

Experience of Home Care of a Patient with Motor Neuron Disease-an Application of Roy’s Model

本篇是描述一位運動神經元疾病個案居家照顧之護理經驗。筆者於民國93年9月27日至93年10月11日，以羅氏適應模式為評估工具，藉由三次電訪及家訪的照護，進行第一層次與第二層次評估，發現個案有：潛在危險性肺吸入、身體活動功能障礙、營養少於身體所需、無望感等健康問題，在護理過程中運用同理心及主動關懷與個案建立良好護病關係後，給予營養照顧的指導，協助復健，並加上家人的支持與關愛，進而增進個案面對疾病的信心及生存的價值。希望藉由此經驗提供護理人員照顧類似個案時，能深入了解其需求，提升個案及家屬對疾病之認識及照顧能力，進而改善其生活品質。

This article described the home care experience of a patient with motor neuron disease. Nursing assessment data was collected through three telephone and home interviews by using the Roy’s model from September 27 to October 11 in 2004. Health problems being identified included risk for aspiration, impaired physical mobility, nutrition less than body requirements, and hopelessness. After building positive nurse-patient relationship through empathy and caring, nursing interventions were provided, including health education about nutrition intake and rehabilitation assistance. Patient’s confidence level has been improved and value toward life has also been reestablished. This article provided the nurses with references of caring similar patients in the future.
