照顧一位鼻咽癌病患之護理經驗

Nursing Experience of Caring a Nasopharyngeal Carcinoma Patient

本篇個案報告為照顧一位鼻咽癌病患，探討其在接受放射治療合併化學治療後產生疲憊、口炎、身體心像紊亂，繼而引發感染等問題之護理經驗，護理期間自92年5月1至92年5月15日，經由觀察、會談、身體評估及運用Gordon十一項健康功能型態評估進行資料收集，確認個案有口腔黏膜障礙、體溫過高、身體心像紊亂、疲憊等主要護理問題，藉由個別性及完善之護理計畫，使個案身心不適症狀能獲得改善，促進其身、心、社會功能狀態及生活品質之提升，筆者藉此個案報告的護理經驗提出與護理人員分享，以助提升日後照護此類病患的參考。

This article reported the nursing experience of caring a nasopharyngeal carcinoma patient who underwent concurrent chemo-radiotherapy and experienced fatigue, mucotitis, body image disturbance, and infection. The patient’s condition was assessed through observation, interview, and physical examination using the Gordon functional health assessment guide and the patient’s medical history. The four major nursing problems being identified included impaired oral membrane, hyperthermia, body image disturbance, and fatigue. Individualized and complete nursing interventions were provided to improve his physical and psychological symptoms, to enhance his functional status, and to promote his quality of life. This report provided a reference for caring similar patients in the future.
