以縫線固定之矽質支架用以治療頸部食道胃吻合洩漏

External Fixation of Silicone Stent in Treatment of Cervical Esophagogastrostomy Leaks

一位經過放療化療的食道癌患者，因為手術後頸部食道胃吻合洩漏，我們使用16 毫米T 型矽質管的6 厘米一段當做食道支架。這患者之吻合口瘺是由於胃近端的前部分壞死。矽質管的近端以兩條縫線固定於頸部皮膚之外。矽質支架五個月後利用食道鏡取出。經過十一個月追蹤，患者可以正常飲食。矽質管支架不僅可以封閉頸部食管胃吻合洩漏，並同時防止吻合處狹窄。

One 6-cm straight segment of 16-mm silicone T-tube was used to stent a major cervical esophagogastrostomy leak after esophagectomy in a patient who underwent neoadjuvant chemoradiotherapy for locally advanced esophageal cancer. The anastomotic leak was caused by anterior partial necrosis of the gastric conduit. Two fixation stitches at the proximal end of the silicone tube were tied over outside the neck skin. The silicone stent was removed by using an esophagoscope 5 months later. The patient was able to swallow regular food at 11 months follow-up. The silicone stent occluded cervical esophagogastrostomy leakage and prevented post-leak stricture.
