50歲女性罹患乳房惡性葉狀瘤合併腦部轉移：罕見病例報告及文獻回顧

Malignant Phyllodes Tumor of Breast with Brain Metastases: A Case Report and Review of Literature

我們報告一位50歲女性罹患乳房惡性葉狀瘤合併腦部轉移的病例，這罕見惡性腫瘤具有高度侵犯性的臨床病程。病患因右側乳房無痛性腫塊約3個月之久而求診，病患並無腋下淋巴結腫大，亦無乳房惡性腫瘤家族病史。經組織病理切片檢查證實為乳房惡性葉狀瘤。在病理組織型態學上，該腫瘤同時包含惡性纖維組織細胞瘤及惡性軟骨肉瘤的樣貌。病患在確診14個月後，因乳房惡性葉狀瘤胸壁復發合併肺部腦部及小腸多發轉移而死亡。

We present a case of malignant phyllodes tumor (MPT) of breast in a 50-year-old woman with an extraordinarily aggressive clinical course. She presented with a 3-month history of a palpable, painless right breast mass without axillary lymphadenopathy and family history of breast cancer. Morphologically, this tumor contained heterologous components including malignant fibrous histiocytoma and chondrosarcoma. Local recurrence occurred 10 months later, followed by metastases involving lung, brain and small intestine. She died 14 months after the initial diagnosis.
