瀰漫性惡性腹膜間皮瘤合併腹內出血及腹膜腔積血：台灣首例報告

Diffuse Malignant Peritoneal Mesothelioma with Intraabdominal Bleeding in a 16-Year-Old Girl: The First Reported Case in Taiwan

我們報告一位16歲女孩罹患瀰漫性惡性腹膜間皮瘤的病例，這惡性腫瘤同時具有不典型的臨床表徵以及罕見的病理組織型態。其中腹膜腔積血在症狀發生初期誤導臨床醫師認為是因外傷而致腹內出血，剖腹探查時發現在腹膜腔及網膜上有許多腫瘤。在病理組織型態上，經證實為惡性腹膜間皮瘤中罕見的肉瘤樣亞型。病患在確診後接受化療，但仍於12個月之後因惡性間皮瘤多發轉移而死亡。

We present a case of diffuse malignant peritoneal mesothelioma (MPM) in a 16-year-old girl with an unusual clinical presentation and a rare histological subtype. The unusual hemoperitoneum misled the physician to diagnose the malignant tumor as intraabdominal bleeding initially. At laparotomy numerous tumor masses and nodules affecting the peritoneal cavity and omentum were found. Histologically, the tumor was a sarcomatoid subtype of MPM. The patient died 12 months following diagnosis in spite of the administration of combination chemotherapy.
