以腹腔鏡處理懷孕合併巨大卵巢瘤之病例報告

Laparoscopy in Ovarian Tumor Complicating Pregnancy

我們報告一個以腹腔鏡處理懷孕合併巨大卵巢瘤病例經驗。一位25歲初產婦，在例行性產前檢查發現左側有一12×10公分的卵巢瘤。在提供相關產前諮詢後，選擇在懷孕十七週以腹腔鏡處理卵巢瘤。病人恢復進食及一般活動情形相當快速良好。雖然在懷孕中腹腔鏡手術有些特定的危險性，因而在傳統上被視為禁忌症，但這種最小侵襲手術方式具有可減少住院天數、減少術後傷口疼痛、減少成癮性藥物使用及快速回復正常飲食等優點，本篇將對此優缺點加以討論。

A case of ovarian tumor complicating pregnancy treated laparoscopically in mid-trimester was presented. A 25 year-old woman, gravida 1, para 0, with a left ovarian tumor measuring 12×10 cm was found during routine prenatal check-up. Laparoscopic surgery was performed at 17th week of gestation. The patient had a rapid, uneventful recovery identical to that of a nonpregnant patient. Though there are risks and concerns specific to laparoscopic surgery in pregnant women, such a minimally invasive surgery has following benefits. It decreases hospital stay, postoperative incision pain, narcotic usage, and shortens the duration to return to a regular diet when compared with open laparotomy. The advantages and disadvantages regarding such an approach were discussed.
