Objectives: By reviewing the return rate of blood transfusion records from 2008 to 2010, we found the rate is lower. Thence Our proposal is the application of cause and effect diagram (fishbone diagram) in order to confirm the main reason, then importing Plan-Do-Check-Action (PDCA) to promote the return rate of blood transfusion records improvement. The return rate of blood transfusion records stood at 89.09% in 2008. It was continued to be improved from 95.96% in 2010 to 99.55% in 2013. Currently Mackay Memorial Hospital adopts Lean Thinking and Methods by the use of cloud derived medical and intelligence decision system to build the real-time and unique barcode identification system. This system includes the monitoring function of the return rate of blood transfusion records, which will replace the traditional way of paper documentation in the future. To establish a better quality of transfusion medical care is always a goal of continuous improvement pursued by blood bank technicians. |
