照顧一位喉癌復發病人接受手術之護理經驗

Nursing Experiences of Caring for a Recurrent Laryngeal Cancer Patient Receiving Surgery

本文為探討一位喉癌復發病人反覆接受手術之護理經驗，照護期間自2013 年3月17 日至5 月10 日，藉由直接照護、觀察、傾聽及會談等方式，運用生理、心理、社會、靈性等方面進行整體性評估，確立主要健康問題有營養狀況改變：少於身體所需/與食慾不振及腹瀉有關、無望感/與疾病進展有關、健康尋求行為/與缺乏氣切造口護理及居家照護相關資訊有關等。透過傾聽、關懷、同理建立良好的治療性關係，針對其個別需求，改善營養及腹瀉問題，循序漸進教導居家照護事項，並引導以正向態度接受自我。希望藉此照護經驗，提供未來護理人員照顧類似個案之參考。

This article discusses the nursing-care experience on a patient with recurrent laryngeal cancer receiving multiple surgeries. From 3/17/2013–5/10/2013, the author interacted with the patient through caring, observing, listening, and discussing. The case was analyzed in physiological, psychological, social, and spiritual aspects in order to identify health problems in the following aspects: Malnutrition, related to poor appetite and diarrhea; Desperation, caused by progression of the illness; and Health-seeking behavior, concerning independent home care. The above problems were addressed through interactions by listening, caring, and establishing a therapeutic relationship. Malnutrition correction, education in home care, and guidance toward a positive attitude were achieved according to individual specific needs. It is hoped that this nursing care experience might serve as a reference for future nursing members with similar cases.
